The form asks for:

Your business name, your name and your email
What you make, what it sells for and about how many items you would bring in
A phone number and links to your work online, if you want to add them
Which you are interested in: a shelf, markets at the shop, or private events

Sign in first, because the application belongs to your Beanchain account. Our answer shows on your dashboard, and we email it to the address you sign in with.
